Getting Started

New Patient/Referral: Expect to arrive 15 minutes prior to your scheduled appointment to fill out new patient forms. Please bring any and all films/CD (X-Ray and/or MRI) for your initial evaluation. Additional X-Rays might be taken in our office if needed.

Re-injury: If you are an established patient of Dr. Pappas and you have re-injured or experienced a new injury, some new paperwork may need to be completed, so please arrive 10 minutes prior to your appointment.

Second Opinions/WC: All records must be faxed to the office and reviewed before obtaining an appointment with Active Sports Medicine. If approved make sure to bring any xrays and/or MRI films/discs to your appointment (unless requested prior). If you do not bring your films we will have to re-xray or reschedule you.

Insurance and coverage: If you are unsure if our office will accept your insurance, please contact your insurance provider with Dr. Jim Pappas, M.D. information. Always keep us informed of any changes to your phone number, address and/or insurance. Covered procedures will vary with each insurance company–our office staff will assist you in determining eligibility. Patients are responsible for deductibles, co-pays and remaining balances.  If you have been referred to us from your primary care physician or belong to a managed HMO program, you are responsible for making sure you have proper authorization and referral forms for all your visits. WITHOUT AUTHORIZATION, WE WILL BE UNABLE TO SEE YOU OR YOU MAY BE RESPONSIBLE FOR YOUR BILL. We will bill all primary and secondary insurances you provide information for. Co-payments are expected to be paid at time of service. The financial responsibility for services rests with the patient or his/her family, regardless of your insurance coverage.

Disability and Attorney forms: For all initial disability and FMLA packets there is a $10.00 fee and then a $5.00 fee for each additional packet.

Physical Therapy: Dr. Pappas communicates with your referred therapist to maintain the progress of your rehab and/or recovery.

Surgery Information/Booklets: Once a surgical decision has been made, appointments will be scheduled for surgery, as well as, pre  and post operative care. You will receive a letter and booklet with surgery information. To obtain any information prior, please call our office.

Medication: Minimal pain medications are prescribed. NSAID’s (non-steroidal anti-inflammatory drugs) are recommended along with physical therapy or surgery as needed.

Medical Records: If you wish to obtain your medical records, contact our office by calling (775) 322-1200. A release form will need to be signed before receiving your records.
